Our Mission
Alianza’s Mission is to empower hard-to-reach, vulnerable, and underserved populations through education and supportive services aimed at working with everyone we serve to achieve better outcomes and higher quality of life.
Our services program complements specialized medical care and brings essential support services to people living with HIV. Intended for those who do not have sufficient healthcare coverage and meet income requirements, this program helps members of our community achieve viral suppression and live healthy fulfilling lives.
Medical Case Management is the initial link to all case management services. A trained and compassionate case manager conducts an initial assessment and will work with the person to create an individualized plan for care, complete with personal goals. The plan may include achieving viral suppression (undetectable status), medication adherence (taking medication as prescribed to maintain a healthy status), or even steps to better communication between clients and their medical providers. Medical case managers also work with clients to enroll them in health insurance and any other programs that can help them cover the costs of medications and treatments.
Non-Medical Case Management concentrates on the provision of advice and assistance in a client’s journey to obtaining social, community, legal, financial, and housing services. These case managers help with transportation for medical care, emotional support referrals, education related to health, food assistance and other resources for healthy and safe living.
Services include the administration of Ryan White programs for financial assistance, health Insurance/Copays/Coinsurance/Deductibles Assistance, and other types of medication assistance and health insurance options are available for clients living in New Mexico.
Those living with HIV can help stop HIV. With the latest U=U (Undetectable=Untransmittable) research, we now know that it is not possible for those with an undetectable viral load to transmit HIV to their partner. Talk to your medical provider about ways for you to reduce your viral load.
